RN Care Manager - Palliative Care



The Palliative Nurse Case Manager (PNCM) is responsible for the care and case management of patient/families with a palliative care consult. The PNCM in collaboration with pt/family, physician and interdisciplinary team, develops and coordinates an individualized Plan of Care (POC) that reflects pt./family treatment goals and symptom management. The PNCM demonstrates clinical competence in pain and symptom management; and fiscal responsibility in the planning and delivery of services. The Palliative Nurse Case Manager will collaborate with Palliative Care MD, Palliative Social Workers, and Practice manager, when implementing POC. All activities will be performed in support of the strategy, vision, and values of Phoebe. For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills. • Wears protective clothing and equipment as appropriate.

Qualification: EDUCATION REQUIREMENTS • 4 year / Bachelor's Degree in Nursing from an accredited college (Required)
EXPERIENCE REQUIREMENTS • 4 - 5 years RN experience in palliative care , oncology, Medsurg, ICU, ER, skilled nursing facilities, hospice or homecare (Required) • 0 - 1 year Minimum of Acute care experience (Preferred) • 0 - 1 year Prefer technical experience with, Oncology, Medsurg, ICU or Homecare and Electronic Medical Record keeping (Preferred) • 0 - 1 year End-of-life and pain management experience (Preferred) • 0 - 1 year Hospice and/or home health experience (Preferred)
PHYSICAL REQUIREMENTS • Have near normal hearing; Hear alarms/telephone/tape recorder/normal speaking voice • Have near normal vision: Clarity of vision (both near and far), ability to distinguish colors
PHYSICAL DEMANDS • Frequently within shift (34-66%) :(Standing,Walking,Sitting)
CERTIFICATIONS AND LICENSURES • Required Certifications/Licensures: Registered Nurse (RN) with current Georgia license • Preferred Certifications/Licensures: Palliative Care certification

JOB SPECIFIC DUTIES: The Palliative Nurse Case Manager is responsible for supporting patients and their families in the palliative care program • Recognizes the patient and family as the “unit of care” and active members in the development of the POC • Utilizes nursing process in providing primary care to pt/ families • Documents all care and care planning per agency policy • Provides nursing interventions for pt/family as ordered by the physician and in accordance with the POC, safe nursing practice and the policies and procedures of PPMH • Assesses and monitors management of pain and symptoms in collaboration with patient/family, IDG and physician to achieve maximum comfort • Exercises good clinical judgment • Demonstrates the ability to articulate the parameters of palliative care services • Demonstrates the ability to work with pt/families under stress & in crisis • Instructs and supports pt./family unit re: symptom mgmt., patient care, and disease process.
CASE MANAGEMENT PALLIATIVE CARE : Documents all contacts/instructions/ interventions per PPMH policies & procedures • Assess patient’s appropriateness for hospice care and refers appropriately • Exercises professional management and coordinates care between and among involved parties in home, SNF , LTAC, inpt rehab setting
INTERDISCIPLINARY PARTICIPATION: Functions as a member of the PCS IDG maintaining open & organized communication with Palliative Care Services staff, volunteers and supervisor • Participates in IDG meeting per protocol to promote problem solving, care planning and learning • Maintains open, organized and timely communication with Palliative care staff • Works collaboratively with IDG to develop common goals to meet the needs of the patient and family • Participates in special projects as requested • Performs other related job duties as assigned by Supervisor
