The R ole: Pre-Visit Chart Review Specialist

Conducts patient chart review to ensure optimal patient care and outcomes prior to visit.

The Opportunity :

MaineGeneral Health is seeking a Pre-Visit Chart Review Specialist to join our growing team! This is a newly created role r eporting to the Chief Practice Officer . T he ideal candidate of this newly created highly collaborative role serves to maximize the outcomes of patient experience and care through the process of pre-visit chart review.

The Work :

  • You utilize systems to identify patients within a specified patient population that require a pre-visit chart review.

  • You c omplete pre-visit planning form on identified patients.

  • You p erform patient chart review to highlight all gaps in care and additional Hierarchical Condition Category (HCC) opportunities.

  • You l iais e and collaborate with a multidisciplinary team of clinical staff and coders across medical practices to ensure gaps in care are closed and HCCs are reassessed and documented.

You Have :

  • Strong organizational skills and ability to handle multiple responsibilities simultaneously and still meet high quality and timeliness standards under pressure.

  • Exceptional attention to detail

  • Excellent interpersonal and team orientation skills that are demonstrated by being articulate, and collaborative.

  • CMA/MA experience preferred.
