JOB DESCRIPTION # Position Title / Job Code: Coder Hospital Based / 8040 Department Name / Number: Health Information Management / 869010 Reports To: HIM Department Supervisor Collective Bargaining Agreement: Service # Support FLSA: Non-Exempt EEO Job Group: Administrative Support Workers Effective Date: Reviewed By: Department Manager Review Date: #4/2/2021 Reviewed By: Human Resources # # JOB SUMMARY # According to ICD-10, CPT, APC coding guidelines, performs analysis, coding, and approvals based on provider documentation in accordance with applicable coding policies for hospital based services re: IP, OBS, ED and Ambulatory coding. # # #PRINCIPLE FUNCTIONS includes the following, other duties may be assigned: # Understands appropriate coding guidelines for Hospital Based Coding with proven abilities to code these services.# Must meet or exceeds coding productivity based on established standards for productivity. Productivity standards must be met for all services coded over six months. Must meet a 95% quality standard based on audits performed for the evaluation period. Must be able to code the following services: Hospital IP, Observation, and Ambulatory Surgery. All records will be coded FOUR days after discharge with the exception of records needing additional documentation or delay in receiving record from a department. Accurately abstract all pertinent information from patient record to include disposition, provider information, diagnostic and procedural information and other data request for data retrieval and qualitative analysis. Query physicians when code assignments are not straight forward or documentation is inadequate, ambiguous or unclear for coding purposes. Verifies medical necessity for Medicare patients receiving services regulated by NCD#s and LCD#s. Monitors the Failed Bill reports to ensure timeliness of coding. Research accounts that have not been coded. Assist in collecting coding productivity data and update coding reports on a monthly basis. Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association (AHIMA) and American Association of Physician Coders (AAPC). Adheres to official coding guidelines published by the American Hospital Association and the AMA for CPT guidelines. Must fulfill all required Education assigned by patient type to coder. # # The following are expectations of all Whidbey General Hospital and Clinic employees: Attends a minimum of 80% of monthly department staff meetings throughout the year. Actively participates in monthly department staff meetings, to include active participation in setting department goals and supporting department functions to meet set goals. Checks hospital email account a minimum of three (3) times per week. Utilizes AIDET in all interactions with patients, employees and visitors. Completes all mandatory education within given timeframe. JOB SUMMARY

According to ICD-10, CPT, APC coding guidelines, performs analysis, coding, and approvals based on provider documentation in accordance with applicable coding policies for hospital based services re: IP, OBS, ED and Ambulatory coding.
