Description
Integrated Care (IC) provides health services including, but not limited to, planned care, rapid assessment clinics, Virtual care and phone advice/triage to support clinical decision making, with a focus on reducing patient length of stay in acute settings, prevent hospital presentations, and bridging the gap between primary and tertiary care.
The medical consultant role is accountable to the Medical Head of Unit and the Director, Integrated Care and is responsible for managing the care of all patients requiring the services of Integrated Care. The position works in partnership with acute, sub-acute, community and primary care teams to ensure patients are navigated through the system based on clinical need. Intervention will be for those assessed as requiring a rapid / time limited intermediate health care response only. Those assessed as requiring inpatient or emergency care will receive care via alternate pathways.
