Job Summary:

The Clinical Liaison will focus on integrating patient care and clinical coordination while leading the development, implementation and ongoing monitoring of programs and initiatives between to clinics and Lumicera health Services.

Essential Functions:

* Work with both clinical and administrative departments to create a collaborative and professional relationship on behalf of Lumicera Health Services and SSM Specialty Pharmacy.
* Serves as the liaison between the medical staff, outpatient pharmacies, insurance providers and patients to improve communication and continuity of care.
* Communicate current and relevant prescription, insurance and financial needs as they relate to appropriate pharmacists, providers, clinical support staff and patients.
* Coordinate and support educational activities as requested
* Support the Supervisor of IDN Clinic Relations to create and deliver educational resources, presentations, and references to enhance comprehension of and participation in workflow processes and procedures that optimize the IDN Shared Services Model.
* Support the Supervisor of IDN Clinic Relations to develop and support the team of the Clinical Liaisons to effectively interface between administrative staff, clinical staff, specialty pharmacies, insurance providers, financial assistance entities, and patients.
* Interacts with clinic employees, including but not limited to pharmacists, clinical staff, fiscal personnel, department managers, access services staff, coding staff, and billing assistants. External contacts include but are not limited to third party payers, manufacturers, prescription benefit management companies, external pharmacy personnel, and referring providers and their support staff.
* Responsible for other duties as assigned by the pharmacist lead/upper management.

* Represent Lumicera and affiliated IDN organizations with a high degree of professionalism in all interactions within scope both inside and outside of the workplace.
* May be asked to perform other duties as requested by Supervisor.

* Participate in, adhere to, and support compliance program objectives.

Requirements:

* Wisconsin or Arizona Employees - Certified CPhT or ExCPT preferred
* High school diploma or GED, Some college preferred
* Minimum of two years pharmacy support experience or healthcare environment experience preferred

* Proficient level of software proficiency in using PC software to support activities.
* Ability to work with confidential material and maintain confidentiality along with sensitivity to employee's needs and data.
* Proficient attention to detail.
* Ability to work scheduled hours, shifts may vary based on department needs. Hours of operation are 8am to 7pm Monday thru Thursday and 8am to 6pm on Friday.

Physical Demands

* Work tasks are performed indoors. Temperature is moderate and controlled.
* Must be able to sit for extended periods of time
* Requires grasping, writing, standing, sitting, walking, repetitive motions, listening and hearing ability and visual acuity.
* Talking and hearing occur continuously in the process of communicating with customers and other employees.
* Vision occurs continuously with the most common visual functions being those of near vision and depth perception.
* Requires manual dexterity to use and operate all necessary equipment.
* All employees must maintain a neat, clean, and well-groomed appearance and dress in a manner appropriate in a professional environment.
