About the role 
Roma, only an hour flight away from Brisbane, the town known for Cattle and its big outback adventures as well as ancient sceneries is not a place to be missed. Our Anglicare Service well known by locals as "the difference in being able to live in my own home for as long as I can" are looking for their next Client Liaison Officer.
Your day to day role will include but not limited to:

  • undertake assessments, support planning, packaged care coordination and case management activities designed in collaboration with the client to assist the client in achieving their identified goals.
  • Supporting the client to manage and monitor their budget to ensure care packages are financially sustainable and
  • monitor the services provided to ensure these are following relevant service models and guidelines.
  • A bout you You will have a background in Community Services, Aged Care or Disabilities.
  • Certificate IV in Health, Community Services, Aged Care or Disability Support
  • Proven understanding of assessments, packaged care and case management
  • Ability to engage clients and work effectively with them to achieve identified goals
  • Support and guidance to clients with budget management and financial sustainability
  • Exceptional communication skills 
  • Ability to advocate strongly for your clients 
  • A sound understanding of the community care system
  • A current National Police Clearance
  • Current Covid Vaccinations 
  • Current Qld Drivers Licence
