About the role
EACH currently has several opportunities available for passionate Wellbeing Case Coordinators! As part of EACH’s Homelessness to a Home (H2H) program, you will support and encourage individuals, couples, and families with complex needs to access and remain engaged with their current support networks and provide support and connections/referrals to expand these networks where possible.
This challenging and rewarding opportunity calls for respectful and collaborative relationships with clients, as you walk alongside them to identify and resolve and/or reduce barriers to achieving their housing, health, and life goals in an effort to prevent a return to rough sleeping.
In this role, you will: Support clients with their social and emotional wellbeing, helping them build resilience and recovery in the areas of mental health, substance use, physical health, and life skills so that they can confidently transition to living in stable accommodation. Use a strength-based approach to the role, looking for opportunities to complement and support existing client strengths and capacities. Work with the client to co-create individual support plans, establishing and maintaining strong, supportive, and professional working relationships with clients. Monitor and respond appropriately to significant homelessness risks, and risks that threaten housing stability. Ensure clients are referred to appropriate supports, engaging other stakeholders as needed to improve clients’ physical and mental health and wellbeing. Provide ongoing support and connections/referrals to expand support networks.
Who we're looking for A self-directed passion and drive to support people experiencing chronic homelessness and/or rough sleeping. A demonstrated understanding of the context and systems associated with housing and homelessness. Strong risk management experience, particularly with vulnerable people who may have complex needs. A tertiary qualification in Human Services, Social Work, or other relevant discipline/profession is highly regarded however not essential.
