GENERAL SUMMARY

The Outreach Supervisor role is dedicated to logistical, field, and direct care responsibilities. They will provide effective leadership by working with or establishing program goals and direct report performance requirements and expectations. Contingent to Statement of Work, the Outreach Supervisor is responsible for the scheduling of outreach activities, team assignments, logistics to support outreach activities to include, vehicle maintenance schedules, outreach inventories, periodic field observations, field supervision and direct care (including linkage, retention, post-discharge follow-ups), and acquisition of PPE. The Outreach Supervisor will plan daily work activities and assign tasks based on skill level, knowledge, and experience focused on meeting programmatic goals. They are responsible for training personnel on tasks required for program success and career development as well as organizing work flow to ensure efficiency resources.

ESSENTIAL DUTIES & RESPONSIBILITIES

* Coordinates access to services as per linkage needs.
* Coordinates diverse activities through communication with staff, key personnel to include internal and external agencies.
* Develops, implements, and enforces the Community Health Worker program's policies and procedures
* Establishes and maintains working linkages through Memorandums of Understanding (MOUs) with a network of community and social service agencies serving or having an interest in the eligible population.
* Establishes direct report performance requirements and expectations and provide timely feedback.
* Organizes work flow to ensure efficiency of applied resources.
* Other duties specified which correlate with program management and implementation.
* Participates on all programmatic conference calls as scheduled
* Plans and provides teambuilding activities held during work hours, and at least once per quarter.
* Plans daily work activities and assign tasks based on skill level, knowledge, and experience.
* Provides effective leadership to a diverse team including Community Health Workers, care managers, linkage specialists and/or care managers by working with or establishing program goals.
* Provides information, referrals, community linkage, and supports to other services and community resources
* Responsible for training personnel on tasks required for program success and career development.
* Routinely reviews Quality Assurance evaluation criteria to ensure compliance with CHCS's expectations for Quality Assurance to include applicable governing directives.
* Spends, at minimum, fifty percent of work time delivering direct participant services to meet contract performance measures.
* Under the guidance of the program director, develops, submits, and maintains quarterly reports and required work logs.
* Performs other related duties as required.

MINIMUM ENTRANCE QUALIFICATIONS

Education and Experience

* Bachelor's degree in Behavioral Science, Counseling or Social Work
* Minimum of 2 years' experience in substance use outreach, intervention, treatment, and/or HIV testing
* Minimum 1 year of supervisory experience
* Meet the competencies to become a DSHS Certified Community Health Worker

Licenses or Certifications

* State of Texas - Licensed Chemical Dependency Counselor, Licensed Master Social Worker (LCSW)

Other Requirements

* Must be able to obtain Texas Department of State Health Services (DSHS) CHW certification within 6 months from start date of employment
* Must maintain a valid driver's license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company's auto insurance carrier if you drive your vehicle during company business.
* Must be able to work in harsh climate conditions.
* Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies.
* Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.

COVID-19 Mandate Requirements

Unvaccinated Staff options:

a. Get Vaccinated per the CMS Mandate.

b. Have a medical/religious exemption. Unvaccinated staff will need to test once a week at the employees own time/expense and must wear enhanced PPE to include an N95 face mask and a face shield.

c. Consumer facing staff DO NOT have the option to be unvaccinated without an exemption.

d. If the position is not consumer facing, staff members may continue to be unvaccinated, without an exemption and test once a week at the employee's own time/expense and must wear enhanced PPE to include an N95 face mask and a face shield.

Vaccinated Staff options:

a. For employees who are vaccinated and consumer facing, we are continuing to require wearing an N95 mask.

b. For all other vaccinated employees, we are requiring a cloth, surgical or N95 mask be worn in all shared spaces.

c. Vaccinated employees may only go unmasked if in a single occupancy office.

PREFERRED QUALIFICATIONS

* Master's Degree

SUPERVISION

* Work requires supervising and monitoring performance for a regular group of employees (2 or more full-time employees) including providing input on hiring/disciplinary actions and work objectives/ effectiveness, performance evaluations, and realigning work as needed. May require supervising up to (2) social work or public health or counseling students.

PHYSICAL DEMANDS

* Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
* Must be able to lift up to 25lbs, stand & walk up to 8 hours a work day.
* Must be able to work in extreme heat or cold for up to 8 hours a work day.
* Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care.
