Position Summary:

This position is responsible for the day to day operations of the growing CCS Care Coordination Team. This includes providing direct management and supervision to the mental health professionals who are directly overseeing the Care Coordination teams. The program manager ensures all contractual guidelines and policies/procedures are being met as well as overseeing quality assurance and improvement. The program manager is the assigned WCS CCS Administrator per DHS 36.

Essential Functions:
  • Monitors and ensures the CCS Care Coordination team meets all relevant local, state and federal standards including DHS 36.
  • Provide direct oversight of all applicable DHS 36 statutes and CCS policies and procedures as the leader of the CCS Care Coordination Team.
  • Support program growth with the goal of expanding the number of Care Coordination Teams
  • Work in conjunction with HR and Director to recruit, screen, interview, and hire new program staff.
  • Responsible for providing direct supervision to the CCS Mental Health Professionals to ensure clinical and day to day supervision is being provided to the Care Coordinators.
  • Monitor productivity of all staff weekly to ensure weekly service hours are being documented and productivity standards are being met.Provide follow up directly with staff as needed.
  • Develop and implement a comprehensive training and evaluation plan for all new Care Coordinators focused on coordinating care, facilitating team meetings, and providing person centered and holistic care.
  • Provide coverage for mental health professionals to include clinical supervision and management to CCS staff.This includes team meetings, individual and side by side direct supervision
  • Closely collaborate with the CCS Ancillary Services Team, including the Ancillary Team Leaders, to arrange for services and provide recommendation on service needs.Work with the ancillary services team to further market and expand services.
  • Collaborate with supervisors, managers, and directors within Behavioral Health Services.
  • Responsible for all CCS specific audits and ensuring all consumer and staff files remain up to date.
  • Responsible for ensuring all documentation is completed by due dates including Recovery Plans, progress notes, service authorizations, eligibility reviews, etc.
  • Ensure all initial and ongoing training for staff is documented and filed accordingly
  • Serve as the CCS Care Coordination Team program representative for interactions with Milwaukee County.
  • Attend meetings and collaborate with community agencies as a representative of WCS
  • Participate and represent WCS in all meetings for the CCS program within WCS and with Milwaukee County Behavioral Health Division
  • Ensures all annual CCS employee evaluations are completed including 90 day evaluation and annual evaluations.
  • Monitors staff documentation to ensure good clinical practice and maximized billing
  • Reviews the CCS budget monthly with Director focused on program growth and sustainability.
  • Proactively interview and hire staff to continue program growth.
  • Ensures adequate staffing of program and provides coverage as needed
  • Assists and supports CCS staff during crisis situations
  • Works collaboratively with others to prepare CCS reports for the county, state, federal or internal needs in a timely manner
  • Responsible for CRC and Agency Demographic Report (compiling information to complete the monthly report, appropriate QA of the information and timely submission)

  • Other Duties and Responsibilities:
  • Submit and approve CCS staff time cards and expense reports in a timely manner
  • Other job-related duties as may be necessary to carry out the responsibilities of the position

  • Job Qualifications:

    Minimal Qualifications:

    Minimum of a Master's Degree in a Social Work or a related field; must have and LPC or be a LCSW, 5 years' experience in the human services field with at least 1 year working with adults diagnosed with severe and persistent mental illnesses; previous supervisory experience preferred. Meet all WCS and contractor requirements for criminal and caregiver background checks, driver's record, and references. Valid driver's license, automobile, and sufficient insurance to meet agency requirements.

    Knowledge, Skills and Abilities:

    Must have advanced substance use , mental health and co-occurring clinical knowledge and skills with the ability to clearly communicate these to others; knowledge of agency policies and procedures and the ability to relay them to others; ability to cope with emergency situations; good supervision skills and the ability to maintain and promote positive working relationships; excellent assessment and interviewing skills; good understanding of mental health and co-occurring needs; ability to support staff in difficult situations; effective use of time and resources to help maintain quality services. Must have knowledge of DHS 36 requirements. Ability to embrace the work of peer specialists.

    Physical Demands:

    Position is mobile with some time spent in the community, including occasional transporting people, home visits and attending meetings; driving throughout Milwaukee County; must be able to go up and down stairs.

    Work Relationship and Scope:

    Reports directly to the Director of Behavioral Health Services Division; works closely with the Supervisors in Behavioral Health Services, the WCS CCS Ancillary Team, embrace the utilization of Certified Peer Specialists and people with lived experience, communicate and collaborate with community based CCS agencies and Milwaukee County.

    Working Conditions:

    Work is performed in the office, the community and in people's homes; people may be in crisis at times; hours average at least 40 hours per week. Flexible work schedule includes some week ends, after hours coverage, and holidays as needed.
