About the Role
The Heart Recovery Service provides an integrated multidisciplinary case management model of care for patients with complex cardiovascular conditions including heart failure.

The role of the Nurse Practitioner Heart Recovery Service will:

• Function in a collaborative model with the healthcare team to provide assessment, diagnosis, treatment and referral as required.
• Work collaboratively with the health care team and contribute to enhanced patient outcomes by actively leading and supporting existing and emerging nurse-led models of care whilst maintaining flexibility and responsiveness to evolving patient and service needs.
• Provide referrals to appropriate agencies, including primary health settings.
