Job Summary and Qualifications:

You will serve in a key role as a Lead Physical Therapist in a beautiful, state of the art, long term care community living setting. You will work in collaboration with a dynamic and innovative multi-disciplinary health care team. The Physical Therapist is responsible for assessing, evaluating, planning, recording and implementing a treatment plan to achieve maximal strength and mobility for improved functional independence for its residents.

Qualified candidates will possess a Degree from an accredited Physical Therapy Program and have a valid Rhode Island Physical Therapy License

Ideal candidate will have a minimum of two years experience in multi-disciplinary long term care setting and preferably in a lead role.

Your Impact

As a Physical Therapist at Bay State Physical Therapy, you will have a direct impact on restoring each individual's maximal function with integrity and compassion and building healthier communities. You will evaluate your patients, develop a plan of care, provide direct care, and help your patients reach their potential. To ensure high-quality care, you will complete daily notes and progress notes in our electronic medical record (EMR) as well as communicate with the patient, family members, and other healthcare stakeholders.
