Description

The Case Manager is directly responsible for supporting the Program Social Worker in the provision and coordination of casework services for resident of the Runaway Homeless Youth TIL Program.

Responsibilities:

  • Provide casework services to residents of the shelter including referrals for entitlements, housing, and services, accompanying residents on appointments and fostering/facilitating relationships with other services providers to meet the needs of the residents of the program.

  • Maintain up to-date case record documentation and reports including progress notes court reports, and all documentation required by city and state governing bodies.

  • Enter case information into DYCDs PTS System.

  • Developing service/treatment plans.

  • Ensure the safety of all young adults on their caseload.

  • Ensure that all residents are in possession of necessary legal documents, i.e., birth certificate, social security card, NY State ID medical insurance.

  • Foster/facilitate relationships with other service providers to meet the needs of the residents of the program.

  • Assist residents with Independent Living preparation, employment, and educational services.

  • Facilitate Independent Living Skills Group workshops for residents.

  • Create curricula binder of resources that focuses on IL Skills such as: budgeting, bank accounts, daily hygiene, cooking, coping strategies, housing, credit, identifying community resources etc.

  • Develop discharge plans and support networks for residents in the program.

  • Provide individual counseling in the facility.

  • Maintain daily communication with Residential Counselors and Program Administrator.

  • Maintain up-to-date case record documentation and reports including progress notes, court reports, and all documentation required by city and state governing bodies.

  • Provide timely communication to DYCD

  • Participate in staff meetings, multi-disciplinary meetings, and planning meetings

  • Other duties as assigned by supervisor

Qualifications:

Bachelors Degree in Human Service-related field. Good writing skills, computer knowledge, clean drivers license.
