* Supervises and provides care to patients.
* Actively uses the nursing process in resident centered patient care, demonstrating accountability as an individual and team member. Synthesizes information from a variety of sources to develop and implement an individualized, age appropriate, collaborative plan of care. Documents the same.
* Provides timely, continuous, age appropriate, collaborative assessment and preparation of patients and family.
* Assesses patients' and families' age appropriate learning needs, coordinates necessary teaching and evaluates teaching outcomes.
* Identifies and effectively communicates pertinent patient information to the health care team, patients and families in a clear, concise and timely manner, from one level or location of care to another.
* Assesses patients' and families age appropriate/discharge needs. Coordinates appropriate referrals.
* Documents and provides to PCC for MDS, assessments and interventions.
* Appropriately delegates and provides follow up to ensure completion of assigned responsibilities.
* Completes physician rounds as needed.
* Performs procedures with minimal supervision. Adheres to policies and procedures in the department manual.
* Demonstrates a knowledge of and utilizes safe and appropriate patient care protocol as set forth in the standards of practice and the unit/department policy and procedure manual relating to the needs of adult and geriatric patients.
* Adheres to professional practice standards.
* Uses problem-solving, communication, time management and collaborative skills to efficiently implement the plan of care.
* Reports all patient and visitor occurrences and completes required documents within 24 hours.
* Maintains confidentiality of patients, visitors, organization personnel and organization business in general.


* Education: Eligible for or currently licensed as an LPN or an RN in the state of South Dakota. Basic Life Support is mandatory within six months of hire.
* Experience: Previous experience in Long Term Care is helpful.
