Do you love working with seniors and making a difference in their quality of life? We may have the opportunity for you...Beatitudes Campus, is seeking an Occupation Therapist for our Medicare Certified Home Health agency that serves our resident clients as well as clients off-campus.


The Occupational Therapist is responsible for establishing, implementing and evaluating goals and objectives for Occupational therapy services that meet and promote the standards of quality of the campus. This position reports to the home health supervisor.

Requirements:

Provides occupational therapy services to patients according to a written physician's plan of care. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates the home environment. Assess for upper extremity strength, mobility, ROM, -potential for rehab. Instructing patients and families/caregivers in the use and care of therapeutic appliances. Determining priority needs for occupational therapy. Reporting to physician, patient's reaction to treatment or changes in condition. Occupational therapists to participate in case conferences. May train patient in use of upper extremity prosthetic devices. Identifies patient and family needs for other home health services and refers as necessary. Prepares and submits clinical and progress summaries based on the attainment of goals. Assures proper maintenance of clinical records in compliance with local, state and federal laws. Participates in discharge planning for patient. Provides occupational therapy consultation to home families/caregivers when indicated. Participates in peer consultation process. Provides in-service education programs for personnel as needed. When therapy is the only skilled service, will instruct, supervise, and evaluate home health aide care every 2 weeks. Stays informed about changes in the field of occupational therapy; shares information with appropriate organization personnel.
