Post Placement Support/Ongoing Support Coordinator

As a Disability Employment Services (DES) provider, we assist people with a disability, injury, or health condition in their journey to employment. Through support, training, assistance and referrals, our aim is to assist our clients to prepare for, find and maintain employment.
Due to significant growth, we are currently seeking a professional and motivated individual to join our team in the position of Post Placement Support/Ongoing Support Consultant . Responsible for the provision of specialised individual support and intervention toclients who have attained employment, you will be required to support the centralised claims team by obtaining compliant documentation for claiming purposes.
Based at our busy Mandurah Office, this position will work with an upbeat and supportive team within the site which is a busy hubs for our staff and clients. Day-to-day the position will be required to
Build strong relationships with clients, employers and other stakeholders
Building rapport with participants to identify, assess and address individual needs
Ensuring participants have attained employment that will lead to sustainable employment outcomes through the development of employment pathway plans and post placement support.
Administration and compliance such as documenting outcomes and managing collection of supporting documents
Essential to success in this role willbe highly developed interpersonal and relationship building skills and demonstrated professionalism, attention to detail and an ability to engage and influence.
Additionally, the successful applicant will be able to problem solve, think outside the box and act as a trusted advisor to our clients and employers.
