Access And Patient Flow Coordinator (After Hours)

About the role

South West Healthcare (SWH) is seeking an experienced senior Registered Nurses for the position of Access and Patient Flow Coordinator (After Hours). This role requires the optimisation and coordination of safe patient flow and effective transition of patients through the acute care setting. The after hour's position provides support to the Emergency Department and Clinical Coordinator by assisting with processes when there is an increase in clinical demand or bed block occurs.
The hours will include Monday to Friday after hour's and weekends 7:00am to 3:00pm.

This is a permanent position, with full time and part time hours available. Please state your preferred hours in your cover letter.

What you bring

The successful applicant(s) will demonstrate a strong understanding of patient flow and demand concepts along with experience in operational planning within the health care environment. They will have the ability to communicate and negotiate with key stakeholders in order to identify opportunities to improve service models and have knowledge of state, regional & local health and community services/networks. Current registration with AHPRA as a registered nurse is essential, a knowledge of local health and community services/networks is desirable.
