Aveo is seeking a Care Team Leader to the lead a team of PCAs within Aveo Freedom Sandringham, VIC.

Reporting into the Care Manager, you will lead and support a dedicated team of personal carers within the community to provide high quality services for residents, ensuring choice and compassion are always supported.

You will set an example for PCAs in your team to ensure that quality individualised care and support follows resident support plans and promotes wellness and enablement.

About the Role:

Assist with the daily coordination and provide support to the PCA team in the delivery of care to residents.
Effective roster management of PCA staff
Supervise personal care staff ensuring adequate orientation and competency assessment
Assist the Care Manager in the recruitment and selection, ongoing professional development and performance of personal care staff.
Ensure that changes in resident needs are escalated to the Registered Nurse or Care Manager
Ensure that daily tasks are completed by the PCA team within the required time frames and aligned to the support plans of the residents
Undertake duties at the direction of the Care Manager and within Scope of Practice
Support residents to maintain independence, dignity, privacy, choice and quality of life

To be successful in this role you will have:

Over five years experience in a lead position in retirement living or home and community care
Certificate 3 in Aged Care or equivalent
Current First Aid & CPR certificate
Current Police Check clearance
Current Drivers Licence with own reliable vehicle
Fully vaccinated against COVID-19

The ideal candidate will possess:

Excellent personal care delivery skills and the ability to participate in care delivery as part of a multi-disciplinary team
High focus on and commitment to customer service, choice and control
Well-developed interpersonal and communication skills
Highly developed interpersonal, customer service and written communication skills
Demonstrated ability to work as part of a team to achieve required care outcomes
Demonstrated computer literacy
