Physiotherapist - Community Response and Reablement Team

Eden Brown are looking for an experienced Physiotherapist to join the Community Response and Reablement Team.

As a Physiotherapist on the team you will provide community based urgent Priority 1 assessments to residents who are referred with undifferentiated, varied and complex needs, ensuring holistic assessment and community care / social care pathway planning is undertaken to directly prevent unnecessary deterioration in function / unnecessary increases in packages of care and prevent hospital admission.

Some of your main duties and responsibilities will be -

  • To assess and treat residents referred for reablement, section 2 intervention and priority 1 referrals.
  • To assess and advise residents on all aspects of their daily life, under the guidance of legislation
  • To be part of a service where your ability to make clinical and social care decisions will aid resident recovery and quality of life.
  • To undertake comprehensive assessments and provide complex strength-based interventions that will have a high impact on clinical and social care decision making and outcomes.
  • You will be expected to work autonomously whilst drawing assistance from other colleagues in primary and social care.

To be considered for this role you will need to:

  • Have a degree in Physiotherapy
  • Be HCPC registered
  • Have a minimum of 2 years post qualifying UK experience
