Position Purpose
Under the direction of the Director of Case Management, the Early Intervention Services (EIS) Program Supervisor supervises all EIS Specialists and coordinates EIS programming/activities to serve individuals living with HIV who are newly diagnosed with HIV or who are not link to HIV medical care. This coordinator monitors medical linkage of individuals who are identified through case finding activities to both primary medical care and social services. The coordinator also conducts and coordinates sexual health educational classes at local organizations/facilities and attends community events to support HIV/AIDS prevention and education to community members. The EIS coordinator manages and participate in outreach for case finding activities.

Essential Functions
  • Implement program activities, policies, and procedures for EIS program.
  • Hire, train, and supervise EIS Specialists.
  • Perform EIS programmatic duties, such as client intakes and continued care, street outreach, testing, and HIV 101 classes.
  • Understand and provide clear and consistent communication around all EIS policies and procedures and grant requirements, including HIV and STI testing.
  • Assist the Testing and Linkage Program manager in organizing information for reporting purposes.
  • Complete chart audits to ensure Quality Management.
  • Collaborate and coordinate services with both internal Vivent Health programs and external community organizations.
  • Collaborate and coordinate services with local hospitals and urgent cares.
  • Coordinate and strengthen services with local incarceration facilities and halfway houses.
  • Implement innovative strategies to reach HIV positive Out-of-Care individuals who are not currently accessing HIV services-specifically, disenfranchised populations such as homeless individuals, recently incarcerated individuals, and MSM (men who have sex with men).
  • Maintain documentation and case notes in the client records.
  • Coordinate and facilitate ongoing HIV 101 education classes.
  • Supervise and support daily activities of EIS staff.
  • In collaboration with the Testing and Linkage Program Manager, assess program activities to determine if objectives are achieved.
  • Participate and collaborate in departmental, agency, and local community meetings.
  • Maintain phlebotomy certification.
  • Maintain regular communication with TLC Coordinator to ensure optimal testing and linkage procedures.
  • Participate in agency events.
  • Perform other duties as assigned.

Required Experience and Skills

  • Bachelor's degree in social work, psychology, counseling, or related field preferred, plus three years of relative experience. Experience may substitute for education.
  • Minimum of two (2) years of experience in conducting street or community outreach or HIV prevention service delivery required
  • Experience working with individuals of diverse cultures, ethnicities, socioeconomic backgrounds, sexual orientations, and gender identities and/or expressions required
  • Understanding of the principles of harm reduction theory and Motivational Interviewing skills
  • Experience working with individuals with substance use and/or mental health disorders preferred
  • At least one year of leadership or supervisory experience required
  • Valid driver's license and personal vehicle required
