Peer Pathfinder

Passages Family Support is recruiting for a Peer Pathfinder. The Passages Peer Pathfinder will work within the Recovery Model of Care, practicing under the Passages’ Standards of Care and under the direct supervision of an MHP. Candidates must be fully vaccinated and must have lived experience with behavioral health challenges. Certification as a peer counselor is required and must be obtained within 6 months of hire. The role of the Peer Pathfinder is to offer peer support services/counseling to individuals who have been incarcerated when they return to their communities. The Peer Pathfinder develops a relationship of trust with the participant. In developing this trust the Peer Pathfinder may function as a role model, peer support, a mentor, a teacher, an advocate, and an ally as they communicate hope and encouragement. The Peer Pathfinder will transition from spending significant amounts of time on social support and begin teaching independent living skills, coping skills and community adjustment skills. Peer Pathfinders work with individuals primarily in their home/residence or in the community. Clients may come to the Passages campus for groups and some individual services, but it is the intent of Passages Family Support to offer in-home individual services as often as possible. Peer Pathfinder engagement activities may include:

  • Interacting with potential participants.
  • Developing a trusting relationship with participants.
  • Promoting a sense of self-direction and self-advocacy.
  • Sharing their experiences in recovery.
  • Helping motivate through sharing the strengths and challenges of their own illness.
  • Considering the Individual’s medical issues and helping them develop wellness plans they can pursue in accordance with their physician recommendations.
  • Helping the Individual plan how they will successfully manage their life in the community.
  • Educating Individuals about resources in their home community.
  • When requested by the Individual join in treatment team meetings if there are no safety concerns. Help to convey the Individual’s perspectives and assist the Individual with understanding the process.

Job Type: Full-time

Pay: $20.00 - $21.00 per hour
