Job Requirements:
  • Perform a comprehensive initial psychosocial assessment and assist in the development and implementation of an individualized comprehensive plan of care
  • Providing services that are ordered by the physician as indicated in the plan of care; 3. Patient, caregiver, and family counseling; 4. Patient
  • Assist in discharge planning of member from inpatient setting./li

  • Work collaboratively to develop and communicate initial impressions, plans of care, changes in care, and progress in a timely fashion
  • Serve as a liaison with social, community, and hospital agencies

  • Participate in training and in-service education and lead training modules relating to SW role in hospice process
  • Educate and assess participants/family understanding and ability to pay Medicaid surplus payment
  • Employ more than 120,000 colleagues at dozens of hospitals and hundreds of health centers in 21 states
  • Provide referrals to patient/family based on knowledge of community resources to maximize overall functions of daily living, and to promote health
  • Collaborate as a key member of the multidisciplinary healthcare team
  • Serve as liaison between medical and nursing staffs, patients, relatives and appropriate outside agencies
  • Provide outreach to identified members through various resources, inbound and outbound engagement of member explaining program overview, benefits and enrollment into the coaching program
  • Complete and submit timely, legible, accurate, relevant and sufficient documentation (written or electronic) addressing the treatment and interventions provided, the and patient's response
  • Adhere to organizational and departmental policies and procedures
