Healthcare Navigator/Case Manager

The Healthcare Navigator/Case Manager is responsible for the assessment of client strengths and needs, development of individual action-oriented goal plans, and coordination of referral and linkage to community resources to assist clients in quickly moving to appropriate housing, and establishing plans for self-sufficiency and stability.  You will participate in the community wide effort to end veteran homelessness and provide services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. Provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties. Work closely with the Veterans primary care provider and members of the Veterans assigned interdisciplinary treatment team.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Work with clients to develop action-oriented goal plans with a focus on securing housing, operating from a “housing first” model.
    1. Implement planning systems that reflect client strengths and needs, client responsibilities and action steps to help clients in achieving their goals.
    2. Provide budgeting and other planning assistance toward the goal of housing attainment and retention, including establishing savings plans based on client income.
    3. Coordinate with Faith Mission Resource Specialists to assist clients in seeking income, employment and housing as appropriate.
  1. Assist clients with referral and linkage to community-based social service and support systems that promote long-term safety, self-sufficiency, and stability.
    1. Refer clients to partner agencies and community resources that benefit their short-and long-term goals.
    2. Interface closely with all Faith Mission staff to provide quality wrap-around services for clients.
    3. Gather information on potential resources for clients in the larger community, creating working relationships and disseminating information.
  1. Maintain clear and precise data and case notes, including timely submission of weekly CQI reports and careful documentation of all client interactions.

  1. Actively participate in weekly Case Review, working with colleagues toward positive client outcomes.
  1. Assist with crisis intervention and de-escalation by providing leadership, and by modeling professional and calm problem-solving behavior for clients, colleagues and the community.
  1. Connect Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care.
    1. Act as a liaison between LSS Faith Mission and the VA or community health providers and works with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans;
    2. Work closely with the Veterans assigned multidisciplinary team, including medical, nursing, administrative specialists, and case management personnel;
    3. Work collaboratively with the team and the Veteran to identify and address systems challenges for enhanced care coordination as needed;
    4. Conduct Non-Clinical Assessments;
    5. Provide ongoing health education support as needed to the Veteran and family member;

  1. Contribute to an atmosphere of safety, sanctuary, dignity, support and resource through adherence to the Faith Mission Code of Ethics and Confidentiality Agreements.

  1. Perform other duties as required.

QUALIFICATIONS

          EDUCATION:

  • Four-year degree in Social Work or related field or equivalent experience required 
  • LSW preferred

EXPERIENCE:

  • Minimum two years working in a social service setting required
  • Experience working with homelessness, mental health or related services strongly preferred

  • Understanding of and enthusiasm for “housing first” model
  • Dedication to excellent customer service, dignity and respect
  • Strong oral and written skills
  • Attention to detail
  • Strong time-management skills
  • Commitment to and aptitude for working in a team environment
  • Ability to lift 25 pounds

OTHER REQUIREMENTS: 

Valid Ohio driver’s license, access to reliable insured transportation and ability to meet LSS insurance standards required
