Case Manager (Community and Social Service)

Agape Coordinating & Consulting, Inc. is a Support Coordination agency that provides case management services to children and adults who have varying developmental disabilities. Case Manager to work with families in need of supports and services through the Medicaid Waiver Program.

Responsibilities of a Support Coordinator

  • Manage full caseload of already established clientele
  • 80% in-office/remote work (telephone calls, related paperwork)
  • 20% on-the-road work (visiting clients in their home/day program/community settings)


  • Bachelor’s degree (in any field)
  • 2 years of experience in any related field of disabilities, case management, or counseling
  • Reliable form of transportation

  • Adaptation to change

Job Type

  • Full time with flexible hours

· Each employee has the ability to work more from home incrementally, after a 90-day training period, and based upon performance reviews and proven efficiency
