We are currently seeking an experienced House Supervisor to coordinate the development and implementation of Supervised Living Services with clients and employees, while ensuring the delivery of quality services.  This coordination occurs with the direct supervision of employees, support for individuals and ongoing communication with family, community, funding and licensure sources.  All coordination must be conducted within contract, state and federal regulations, as well as company policies and procedures.  This position reports directly to the Program Manager. 

Work Hours

Days, evenings, nights, weekends, and 24-hour on-call availability for emergency response.

Responsibilities

  • Person Centered Planning and Individual Support Planning:
    • Participate in and monitor the implementation of Person Centered Plans (PCPs), the PSS and Crisis Plan
    • Design and implement support and teaching strategies targeted at assisting individuals with reaching their planned outcomes
    • Provide supervision and training to Direct Support Professionals regarding the implementation of the PCP, PSS, and teaching strategies
    • Evaluate and revise support and teaching strategies, as necessary, to ensure progress toward achievement of outcomes and action steps
    • Revise PCP outcomes, as necessary, based on data collection, feedback from staff, and direct observation of the individual
    • Ensure that services and support are provided in the PCP and PSS Plan
    • Complete monthly outcome reports and provide to the Program Manager
    • Facilitate opportunities for natural support

  • Consumer Record Maintenance of individual files including, but not limited to:
    • Individual profile data with emergency contact numbers
    • Copies of Medicare/Medicaid cards for all supported living sites, current Release of Information forms, Consent forms, and current signed Consumer Rights statement
    • Copies of guardianship paperwork, if applicable
    • Financial records in the Help at Home/Oxford HealthCare Inc. financial system, including documentation of all account spending, personal spending, and household spending
    • Daily progress notes, monthly reviews, and other client-related documentation
    • Medical information including documentation of necessary medical appointments, lab results, immunizations, physicians medication orders, Medication Administration Records and controlled drug count sheets
    • Monthly Health Summary
    • Monthly Fire Drills
    • Ensure that nutrition guidelines on the PSS and the Crisis Plan are followed
    • Ensure that grocery and household shopping is done
    • Complete monthly Safety Checklist

  • Quality Assurance-related functions:
    • Maintain confidentiality of all individuals' Protected Health Information, as mandated by HIPAA
    • Review all forms, notify Case Manager of any incidents, and provide oversight for the follow-up plan with staff
    • Maintain communication with family members, guardians, staff and Case Managers on a regular basis, particularly regarding policies, issues, and concerns
    • Prepare and submit reports that are required by contract, funding sources, or regulations      

  • Supervision/Training:
    • Coordinate in-service training that is needed to assist staff in fulfilling responsibilities related to client support
    • Provide oversight with documentation of the Medication Administration Record with checks of the MAR at least once per week
    • Monitor and record all required training for staff and schedule updates
    • Complete a monthly staff schedule, ensuring appropriate levels of staff coverage
    • Provide direct support in providing services needed to protect the health and safety of the client, especially in a staffing crisis
    • Provide training and oversight for staff regarding Medicaid Waiver guidelines and licensing and certification standards
    • Communicate with the Interdisciplinary Team about concerns and findings
    • Report personnel issues to the Program Director/Executive Director
    • Be available to respond to staff in case of an emergency
    • Perform On-the-Job Training (OJT) for all support staff
    • Perform on-site evaluations for all field staff
    • Complete all annual evaluations and submit to Program Director/Executive Director 

  • Other job functions:
    • Coordination of individual benefits including Medicaid, Medicare, SSI, SSA, and Food Stamps
    • Secure community resources for individuals supported
    • Maintain individual files, per licensure and company regulations
    • Ensure compliance with licensure guidelines
    • Coordinate client medical needs
    • Assist the Program Director/Executive Director in teaching the training classes, as requested
    • Assist the Program Director/Executive Director with on-site investigations, as requested, for worker injury and client complaints
    • Assist support staff with other duties, as assigned and with approval

Minimum Qualifications

  • Bachelor's degree in social work, psychology, human services or a related field OR at least one year experience in the field of developmental disabilities
  • Supervisory experience preferred
  • Personal vehicle which is insured and meets state safety/licensure requirements
  • Valid driver’s license, may require chauffeur’s license
  • Availability to work day, evening, night and weekend shifts
  • Availability to work PRN shifts
  • Availability to be On-Call

House Supervisors must comply with state background screening requirements. Compensation, benefits, time off and bonuses vary by state and location, so please ask for complete details at your interview.

Caregivers will be eligible to receive the bonus of $500 provided they remain active for at least 30 days after hire and have worked 40 hours.  Bonuses will be paid within 30 days of reaching eligibility. New caregivers as well as previous caregivers who have not worked for Help at Home since prior to 1/1/2021 will be eligible for the bonus. Caregivers who are transferring from an organization within the Help at Home, LLC portfolio of companies are not eligible to receive the bonus.
