Social Worker

Job Summary:
Provides professional social work services in the acute care hospital setting, utilizing supportive counseling through biopsychosocial assessments. Implements clinical knowledge in assessing the needs of patients and families during illness. Provides crisis intervention as patients and families transition to post discharge planning. Collaborates with members of interdisciplinary health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community.

Minimum Requirements:
• Graduate Degree Master’s degree in Social Work or LMSW from accredited program 
• Organization, customer relations & verbal communication
• License to practice in Social Work in the state of Kansas

Key Responsibilities:
• Maintains confidentiality at all times.
• Provides a well-thought out decision or response; obtains and analyzes all pertinent information available in order to make the most informed decision/response based on factual and objective data.
• Works cooperatively with supervisor and those in leadership positions. Supports department expectations and participates in departmental activities, e.g. staff meetings, education, projects, and communications. Cross trains to duties to support the department.
• Demonstrates a consistent level of performance; maintains progress on work assignments and meets deadlines. Seeks guidance as necessary for performance of duties.
• Demonstrates appropriate and effective conflict resolution skills. Address issues with individuals involved in a courteous and professional manner and escalates issues to leadership as necessary.
