Ocean Health Initiatives, Inc. (OHI) is a Federally Qualified Health Center (FQHC) dedicated to providing quality, accessible and comprehensive primary health care to the residents of Ocean and Monmouth County; regardless of economic status.


Our Health Center Locations: Brick, Freehold, Lakewood, Little Egg Harbor, Manahawkin, Manchester and Toms River; with our school-based Wellness Programs located within the Clifton Ave Grade School, Lakewood; and Lakewood High School.

OHI services include family and internal medicine, pediatrics, behavioral health, nutrition, OB/GYN women’s health, dental, family planning, specialty care, pharmacies, PrEP, and a STI clinic.

Hours:
Full-time hours are M-F 8:00am- 4:30pm with rotating evening/Saturday shifts (subject to seasonal changes and business/site needs)

Position Summary:

Reporting directly to Director of Nursing (DON) and Practice Director (PD) the Licensed Practical Nurse performs care including; telephoning patients, immunization administration. Plans, coordinates and evaluates services utilizing the direct nursing process, comprehensive nursing care and individual case management.

Responsibilities:

  • Provides nursing care including: administration of medications and injections to patients as directed by Providers, immunizations, referrals to other providers and community health resources, preauthorization’s as required for any testing and health instructions to patients following examinations.
  • Practice professional attitude and attire at all times, clean and neat uniform, short fingernails, no large jewelry of any type for safety and infection control best practices and no facial piercings.
  • Utilizing the nursing process, LPN will provide direct and in-direct patient care by identifying patient problem and concern and report to provider for further evaluation.
  • LPN will complete initial/new patient intake by completing patient past and current medical diagnosing, concerns, medications and take patient’s vital signs, including blood pressure, respiration rate, pulse, pulse oximetry, height and weight, and obtain patient history on smoking, depression, allergies and any health concerns or limitations. Document all information utilizing a computer or Laptop in EMR.
  • Assist provider in patient education on smoking cessation, healthy diet and health lifestyle choices
  • Educate patients on the importance to take off shoes to obtain accurate weight, and inspect feet, especially on diabetic patients.
  • Assist providers with providing on-going education and disease management for chronically ill patients (ex. Hx of Diabetes, asthma).
  • As a part of uniform requirement, LPN must wear hand-watch with second hand in order to provide safe and quality patient care and obtain accurate vital signs.
  • Reviews level of pain with patient and documents all vital signs in patient’s chart in the EMR.
  • Monitors patient flow and, in the case of patient overflow, assists CMA in preparation of examination rooms, chairs and preparation of patients for exam. Effectively work with a care team in order to deliver safe and quality patient’s care.
  • Administers vaccines according to policy using two identifiers, providing proper education and information sheets to patients/guardians, and accurately documents information in chart overview of EMR.
  • Enters appropriate patient data and all the vaccine information in NJIIS Registry right after the vaccine is administered.
  • Enter refrigerator temperature appropriately and in a timely manner in NJIIS as required by VFC
  • Maintain an accurate, and daily temperature log for refrigerator thermometer to assure vaccine and medication safety. Update the log on a monthly basis, file the log for previous month and send a copy to DON by 5th of each month
  • Inspect and check medications/vaccine expiration dates on a continuous basis, and maintain the expiration medication log
  • Clean refrigerator on a monthly basis, and maintain an up-to-date refrigerator cleaning log
  • Follow up with a telephone call when patient was sent to ER and document in EMR promptly
  • Possess good time management skills, and ability to multi-task in a fast-paced environment, while maintaining nursing competency and patients confidentiality
  • OB and PEDS LPN inquires about patients dental needs and assist the patient to set up a dental appointment
  • Direct for SW assessment all patients with hx of depression, suicidal thoughts or high risk patients
  • Be detail-oriented, make rounds , supervise /guide and mentor CMA to achieve best patient care
  • Maintains adequate PAR levels and works with DON to adequately supply routine medical supplies; re-orders supplies through established purchasing procedures on a monthly basis or an needed, and keep all necessary logs updated. Send copies of all logs to DON on a monthly basis
  • Maintains updated file of referrals and provides referral information to Providers.
  • Provides direct and/or indirect patient care oriented toward improving or maintaining health and preventing illness.
  • Observes patient, records significant conditions and reactions, and notifies Provider of patient's condition and reaction to drugs, treatments, and significant incidents.
  • Initiates a patient education plan according to the individualized needs of the patient, as prescribed by Provider and OHI policy including patient and family instruction.
  • Reconciles labs within normal limits, reports critical labs and abnormal results to Provider following OHI policy.
  • Manages organizer effectively and daily, completes medical calls in the organizer in a timely manner, pharmacy refills, pre-authorizations as necessary. Report to DON any questions, and concerns.
  • Complete various forms as requested by a patient and report to provider for final review and management.
  • Reports required information to the Health Department for further screenings and follow-up.
  • Ensures chart preparation is completed on a daily basis, assigning necessary tasks to the Certified Medical Assistants and forwarding assignments to and shift reports to the DON and Practice Directors daily.
  • Maintains and updates patient charts, records, and information via EMR.
  • Anticipates any safety concerns in the environment of care and intervenes as necessary.
  • Reports all necessary information and/or unusual occurrences in accordance with established policies and procedures, and notifies DON/Practice Director of unusual occurrences as they develop.
  • Completes incident reports immediately as per policy in all required situations.
  • Reports action of others which have negative impact on patient care.
  • Promotes and delivers optimum satisfaction to all patients.
  • Attends and completes all mandatory in-services and attends required meetings.
  • Works collaboratively with all members of the Senior Management and Management Teams to resolve departmental and organizational challenges expeditiously, economically, professionally and with minimum disruption to the practice.
  • Orients and trains staff as observed by DON or designee.
  • Develops and maintains good human relations skills and effective communications by promptly notifying providers, DON and other involved clinical team on change in patients condition, new orders and other pertinent to patients care information.
  • Ensures that No Show patient is called by CMA within 30 min after the appointment is missed, assist patient with re-scheduling the appointment and document accordingly.
  • Participates in morning huddles with clinical teams and PD’s and coordinates all clinical activities in the Department and daily staff assignments and keeps log.
  • Displays a respectful, caring attitude toward patients at all times.
  • Accepts constructive criticism as demonstrated by positive behavior changes.
  • Adheres to Corporate Compliance policies.
  • Notifies the Director of Nursing and Chief Medical Officer on matters relating to nursing practices.
  • Reports any patient or staff issues to DON, CMO and PD.
  • Provides input to DON in reference to development of policies and protocols for patient care.
  • Prepares and sends daily shift reports to the Practice Director and DON.
  • Provides leadership in the development of standards of care for the Department, patient/family education and staff education to address the needs of specific populations.
  • Guides the professional growth of others through role modeling, coaching and/or mentoring.
  • Provides leadership at a unit level for initiatives designed to improve the quality of care or clinical outcomes for populations of patients and growth of others.
  • Consistently demonstrates The Code and ensures all clinical roles engage in this practice.
  • Participates in ongoing quality initiatives.
  • Participates in OHI collaborative quality improvement activities and develops Plan-Do-Study-Acts.
  • May cover any OHI sites as needed.
  • Other duties as assigned.

Education and Experience:

  • Graduate of accredited Licensed Practical Nursing program
  • 1-3 years nursing experience in medical center or community health care settings.

Licensure:

  • Active State of New Jersey LPN license is required.

Certifications:

  • Basic Certified Life Support is required. Maintains CPR certification as evidenced by valid card.
