Job Description/Requirements:

The Physician Verification Specialist will provide a high level of organization and effective completion of physician verification workflow and resolution coordination. This position will be accountable to the Internal TCC and will function collaboratively with all other employees and across departments to ensure that all patients referred for care have a physician for the oversight of their home health episode.  This role will also assist with new referrals for home physician or PCP consultation.

 Education:

  1. High school graduate or equivalent
  2. Some college or associate degree preferred

 Skills/Experience:

  1. Some experience in health care data entry preferred.
  2. Use of Microsoft Office Suite including Word, Excel, SharePoint
  3. Keyboarding efficiency of 50 WPM or greater
  4. Strong verbal and written customer service skills
  5. Ability to problem solve
  6. Excellent organizational skills
  7. Very high level of attention to detail
  8. Efficient with word processing, data entry and telephone communications with basic medical terminology

 Essential Job Functions / Responsibilities

The Physician Verification Specialist will be responsible for:

  1. Reviewing referral information and transitional care coordination documentation to verify each patient’s following physician prior to the point of agency admission.
  2. Providing guidance for troubleshooting of patients who do not have a confirmed physician to oversee their home health care services.
  3. Providing new outside referrals for a PCP or home physician when a verified physician does not exist for a patient.
  4. Providing prompt feedback to TCC team members regarding status of physician verification in order for resolution of outstanding or non-compliant documents and information
  5. Serving as a resource for other departments when entering, verifying, or contacting physicians regarding PECOS/Medicare enrollment, as well as status as a certifying physician.
  6. Providing patient referral leads to Internal TCC for physician offices who either do not utilize HRS services or do so at a low rate.
