Residential Home Health and Hospice is seeking Registered Nurses with supervisory experience to join our home health team as a Home Health Clinical Supervisor.

Home Health Clinical Supervisor Responsibilities:

  • Manage the assigned home health clinical care team and assist with care coordination and oversight.
  • Train, mentor, and evaluate home health clinical care team on processes, protocols, compliance, and documentation requirements.
  • Monitor clinical records to ensure visit, quality, and satisfaction guidelines are met.
  • Serve as liaison between field staff, referral sources, and home health operations.
  • Evaluate and address patient, caregiver, and customer concerns expeditiously.
  • Monitor visits that require supervisory oversight including home health aide and licensed practical nurse visits. 
  • Regularly audit and review patient charts to ensure proper and complete documentation and provide constructive and timely feedback for improvement.
  • Communicate company-wide changes to staff and provide support and guidance when needed.
  • Analyze required supervisory reports and document utilizing Homecare Homebase (HCHB), Residential’s EMR, through a company-provided tablet.
  • Attend all required staff meetings. 

Home Health Clinical Supervisor Qualifications:

  • Must hold, in good standing, a license as a Registered Nurse issued by the state in which you work.
  • A minimum of two years working as a Registered Nurse in a clinical care setting, hospice or home health environment preferred.
  • Minimum of one-year supervisor or management experience.
  • Must have strong communication and organizational skills and high standard of ethics and compliance.
  • Ability to document within an electronic medical record.
