HIM Associate II - Medical Records

The HIM Associates I. II, III processes health information under the direction of the HIM Director or designated supervisor. This processing includes, but is not limited to: collecting and/or delivering health information/hard copy medical records for patient care; retrieval of medical records from on-site and off-site storage areas; pick-up of discharged patient records from nursing units; preparation of documents for storage via scanning or boxes; preparation of records for document imaging; prepping of documents in the medical record; release of health information; answering telephones; and/or assisting visitors, patients, physicians and ancillary staff with health information requests.

 

Level I

· Processes incoming medical records from off-site storage company and routes medical records to appropriate location.

· Picks up and organizes all discharged patient medical records daily.

· Accounts for discharged patient medical records (inpatient and/or outpatient) daily; follows up each day thereafter until medical record is located; notifies supervisor if medical record is missing greater than 3 days.

· Distributes mail to appropriate location in HIM department: separates out release of information requests.

· Accurately performs computer look-up on Affinity system; is able to discern location of medical record based on discharge date; type of service, and record processing cycle.

· Answers telephone and responds according to departmental policy/procedures.

· Seeks out additional tasks when assigned work is completed; provides backup coverage for other positions in the HIM department.

· Keeps supervisor informed of issues/concerns that may interfere with work assignments being completed timely; provides production/activity logs without prompting.

· Secures and protects health information according to hospital and department policies.

· Meets production standards on a consistent basis.

· Performs other duties as assigned

 

 

Level II

  • Accurately scans documents; resets image properties to enhance scanned image; minimal rescans required.
  • Accurately analyzes records for deficiencies: determines items requiring completion; assigns correct physician. Re-analyzes medical records for completion; works rejection charts and reassigns as necessary.
  • Accurately performs computer look-up on Affinity system; is able to discern location of medical record based on discharge date; type of service, and record processing cycle.

  • Answers telephone and responds according to departmental policy/procedures.

 

 

  • Seeks out additional tasks when assigned work is completed; provides backup coverage for other positions in the HIM department.
  • Keeps supervisor informed of issues/concerns that may interfere with work assignments being completed timely; provides production/activity logs without prompting.
  • Secures and protects health information according to hospital and department policies.
  • Follows Operative/Procedure reports process according to department policy.
  • Monitors and reviews transcription vendor interface.
  • Works transcription pended queue and ensure reports are distributed in a timely manner.
  • Works with physicians and hospital departments to resolve transcription errors and issues.
  • Provides feedback to the transcription vendor on a timely and regular basis.
  • Performs other duties as assigned

 

 

Education, Training and Experience

 

Required: Level I- High School diploma or equivalent.

Level II- High School diploma or equivalent. One (1) year medical record experience. Knowledge of medical terminology and anatomy.

 

Skills or Other Qualifications

 

Required: Basic keyboarding skills. Ability to withstand the pressure of continual deadlines and receipt of work with variable requirements. Ability to concentrate and maintain accuracy despite frequent interruptions. Ability to be courteous, tactful and cooperative throughout the work day. Ability to use standard office equipment including computers, photocopy, and facsimile (fax) machines and microfilm reader/printers.

Preferred: Ability to process work using both alphabetical and numerical filing systems. Must be well organized and demonstrates an aptitude for accuracy and attention to detail; demonstrates effective communication, interpersonal skills, and knowledge of proper body mechanics, safety measures and infections control, and ability to follow instructions.
