Assignment details: Contract length: 13 weeks Shift length: 13 hour shifts Hours per week: 36 Start Date: ASAP Job responsibilities: Provide patient and family education related to the plan of care and discharge needs. Coordinate patient care activities with physicians and other clinical team members. Document assessments, interventions, and patient progress in the medical record. Support safe transitions of care, including handoffs and care plan updates. Work 36 hours per week with 13-hour shifts.
