Home Health Baylor Registered Nurse

CenterWell Home Health is seeking a compassionate and skilled Home Health Baylor Registered Nurse to provide high-quality, in-home nursing care to adult and geriatric patients over concentrated weekend Baylor shifts. In this role, you will conduct comprehensive patient assessments, develop and implement individualized plans of care, deliver skilled nursing treatments, and coordinate services that support patient autonomy and safety in the home setting. You will educate patients and caregivers on disease management, medications, and home care techniques, advocate for patient needs, and collaborate closely with physicians, therapists, and the broader interdisciplinary care team to ensure continuity and excellence in outcomes. The Baylor schedule offers a focused weekend work pattern, typically working designated weekend days and hours in exchange for full-time pay and benefits, providing both schedule predictability and work–life balance. As a CenterWell nurse, you will document accurately and promptly in the electronic medical record, complete OASIS and other required assessments, monitor changes in condition, and respond quickly to urgent patient needs. You will practice according to all regulatory standards and CenterWell policies, upholding a culture of compassion, integrity, patient safety, and continuous improvement.
