LifeBridge Health is seeking a compassionate and skilled Nurse Practitioner to join our Healthcare & Medical Services team within our Healthcare & Medical Services department. In this role, you will independently assess, diagnose, and manage acute and chronic health conditions for a diverse patient population, working within your full scope of practice and in close collaboration with physicians, nurses, and other members of the interdisciplinary care team. You will obtain comprehensive health histories, perform focused and complete physical examinations, order and interpret diagnostic tests, and develop evidence-based treatment plans tailored to each patient’s unique needs. The Nurse Practitioner will prescribe medications and therapies as permitted by Maryland state regulations and LifeBridge Health policies, while closely monitoring outcomes and adjusting care plans to optimize patient health.
You will play a central role in educating patients and families about diagnoses, medications, self-management strategies, and preventive care. A strong emphasis is placed on chronic disease management, including conditions such as diabetes, hypertension, heart failure, COPD, and other common medical issues, as well as the management of acute and urgent concerns. You will promote wellness and preventive health through routine screenings, immunizations, and personalized health counseling, reflecting LifeBridge Health’s focus on proactive, community-oriented care.
Collaboration and communication are essential components of this position. The Nurse Practitioner will work closely with attending and consulting physicians, bedside nurses, care managers, pharmacists, therapists, and social workers to coordinate care across the continuum—from outpatient visits and specialty referrals to hospitalization, discharge planning, and community follow-up. You will document all encounters thoroughly and accurately in the Electronic Health Record, ensuring clear, timely, and legally sound documentation that supports seamless transitions of care and quality reporting.
