Pay Range: $26.06 - $36.08

This position functions within SEARHC Community Wellness and is responsible for coordinating the delivery of appropriate and timely care for patients, promoting quality and cost‑effective health care outcomes, and ensuring efficient daily operations of the Nutrition Services Team, WISEWOMAN Women’s Health Program, and Diabetes Education Team.
Key Responsibilities
- Coordinate safe, effective, equitable, patient‑centered care.
- Assess patients’ and caregivers’ self‑management skills and encourage wellness and autonomy.
- Communicate regularly with patients and caregivers regarding referrals.
- Prioritize patient referrals, outreach, and coordination of care to maintain safety and quality.
- Support care coordination within and outside the SEARHC health system for consistent, effective service.
- Track and maintain case‑management coordination data identified by program managers.
- Schedule patient appointments, manage calls and queries, and communicate via WellApp messaging.
- Generate patient letters and complete relevant forms.
- Advocate for patients and caregivers, empower decision‑making, and address needs timely.
- Collaborate with Community Wellness staff to establish care plans that maximize health outcomes.
- Verify patients’ insurance profiles and demographics, updating as needed.
- Participate in departmental and clinical improvement efforts and maintain a clean, safe environment.
- Foster teamwork and utilize resources effectively.
- Provide administrative coverage for scheduling and staff support.
- Monitor and update schedules for Nutrition Services, WISEWOMAN, and Diabetes Education Team.
- Ensure accurate pre‑registration and insurance eligibility for all scheduled patients.
- Coordinate educational in‑services provided by Community Wellness providers when needed.
- Maintain high levels of customer service, professionalism, and courtesy toward patients and staff.
- Collect and enter required data into designated databases for consortium communication.
Qualifications
- High School Diploma or equivalent.
- Associate’s degree or 2 years of college in social work, social services, healthcare administration, business administration, or related field.
- 4 years of relevant professional experience may be exchanged for a degree.
- Minimum of two years’ experience in a healthcare setting – required.
- Primary care or specialty clinic experience preferred.
- Knowledge of medical terminology.
- Proficiency in using electronic health records (EHR).
- Understanding of insurance plans and policies.
- Computer literacy.
- Problem‑solving and conflict‑resolution skills.
- Strong interpersonal, organizational, and multitasking abilities in a fast‑paced environment.
