Calvary’s proven treatment programs address the practical, physical, and spiritual aspects of addiction for lasting recovery. Our flagship location in Phoenix offers on-site detoxification and residential treatment programs, a partial hospitalization program (PHP), and an intensive outpatient program (IOP). We also have a satellite clinic offering Intensive Outpatient Programs in the West Valley region. Case Manager Position Summary

The Case Manager works under the guidance of the Clinical Director while mentoring case management staff. The Case Manager collaborates with other healthcare team members to coordinate and facilitate the effective delivery of quality services to Calvary patients. The Case Manager is knowledgeable about and responsible for treatment plans, utilization review, discharge planning, psychoeducation groups, and case management.

Qualifications

  • Bachelor's degree required, Master's degree preferred.
  • Minimum of 2 years of experience in case management related work.
  • Valid Fingerprint Clearance Card.
  • Completion of BLS and First Aid Training within 30 days of employment.
  • Completion of Verbal De‑Escalation Training within 30 days of employment.
