Job Summary

Care Managers serve as collaborative partners in patient care, working with patients, caregivers, nurses, social workers, physicians, and community resources to facilitate communication and coordination across the continuum of care. Our goal is to achieve optimal health outcomes, advocate for patients, and ensure effective transition and utilization of resources.

Key Responsibilities

• Assess patients, plan and implement individualized care interventions
• Coordinate and monitor care across providers and settings
• Communicate with patients, families, and post‑acute care partners to ensure needs are met in the community
• Document and evaluate care plans and outcomes

Required Qualifications

• Registered Nurse licensed in California
• Bachelor’s Degree in Nursing (or obtain within 3 years of hire)
• Minimum 2 years of experience in a health‑care setting (acute, ambulatory, post‑acute, etc.)
• Minimum 2 years in case management or successful completion of the Transitions in Practice (TIP) program
• California RN license on hire

Preferred Qualifications

• Master’s Degree in a health‑care related field

Pay Range

$56.34 – $85.55 per shift; additional compensation may include shift differentials, standby/on‑call, overtime, and bonus opportunities.
