Compensation & Benefits
Compensation range $85,000 - $95,000.
Paid Days Off 30 days annually – up to 6 weeks (including company holidays).

Responsibilities
- Perform skilled nursing visits and coordinate client care.
- Act as point of contact for all disciplines involved in patient care.
- Oversee frequency of visits for the episode.
- Consult with the physician and office, providing patient care details.
- Perform accurate OASIS collection, ensuring medication profiles remain current.
- Ensure lab values are reported to the physician timely.
- Attend weekly case conferences and monthly case manager meetings.
Qualifications
- Licensed Registered Nurse (RN) in the state of employment.
- Minimum of one year of clinical experience preferred.
- Demonstrated knowledge and skill in current nursing practice.
- Valid state driver’s license and automobile liability insurance.
