Responsibilities
- Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education, and referrals.
- Administer and order point-of-care and lab-based testing as needed.
- Collaborate with physicians, case managers, social workers, and family members.
- Provide patient education and follow-up in collaboration with the Primary Care Physician (PCP).
- Deliver services at the member’s residence, including homes and Skilled Nursing Facilities.

- Competitive Pay – Earn income based on assessments completed. Monthly averages of 20–100 visits earn $1,800–$11,500, per assessment rates of $90–$115 depending on state.

Qualifications
- Master’s Degree or commensurate experience and NP licensure.
- Current NP licensure with prescription authority or ability to obtain it.
- AANP, AACN, or ANCC board certification in Adult Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.
- Current BLS, ACLS, or CPR certification.
- 1 year of NP experience preferred; new graduates encouraged to apply.
- Valid state driver’s license and proof of adequate automobile insurance coverage.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
