Home Based Health Specialist
Provides administrative support in the health area to staff in Prenatal and 0‑3 Early Childhood programs. Monitors documentation of children’s health data and enters information in the data management system, including updates to health files and entries into CP and Salesforce. Schedules health services for children in program and reports to the Health Coordinator, Home Base Supervisor.
Candidate Qualifications
- Associate’s degree in public health, social services or related field required; bachelor’s degree preferred.
- Demonstrated experience with database management required; experience maintaining health data a plus.
- One year experience working with children preferred.
- Bilingual (English/Spanish) required.
- Valid driver’s license, auto insurance, and daily access to reliable transportation required.
Job Responsibilities
- Schedule hearing, vision, and oral health screenings for children throughout the year.
- Create and update the schedule of annual health screenings; distribute the schedule to staff as directed.
- Enter health data in the data management system for clients served, including physical exams, well‑baby check reports, hearing and vision screenings, immunizations, and oral health exams.
- Complete monthly reports of services provided and scheduled for the sites.
- Report to the Health Coordinator regarding missing health documentation on a monthly basis or as needed.
- Collaborate with the nurse and nutrition consultants to schedule regular monitoring of children’s files.
- Regularly review children’s files to ensure accurate documentation of health requirements and alignment with data in the system.
- Coordinate training for staff and parents on health needs of children under five.
- Manage logistics for Health Service Advisory Committee meetings twice per year.
Job Details
- Compensation: Hourly rate of $18.50–$20.00 per hour; bilingual candidates may receive additional compensation. Salary will be determined based on experience and qualifications.
