Registered Nurse – Community Palliative Care

Make a difference in community end‑of‑life care. Employment Type: Permanent Full Time, 38 hours per week. Salary: $74,317.94 – $104,336.27 per annum + 12% super + salary packaging. Location: Gloucester.

Overview

As a Palliative Care Registered Nurse you will conduct home visits, provide specialist symptom management, case coordination and medication management. You will work with the Lower Mid North Coast Specialist Palliative Care team to address patients’ physical, emotional and social needs, and be part of the Generalist Care multi‑disciplinary team that includes Community Health Nursing, a Nurse Led Clinic, Child and Family Services and other outreach services.

Responsibilities

  • Carry out home visits within the community providing Palliative Care Nursing to patients.
  • Deliver compassionate, person‑centred care for individuals with life‑limiting illnesses.
  • Provide specialist symptom management, case coordination and medication management.
  • Work collaboratively with the Specialist Palliative Care team and other multidisciplinary team members.
  • Prioritise caseload, coordinate clinical activities and communicate effectively with patients, families and staff.

What We’re Looking For

  • Current authority to practice as a Registered Nurse with AHPRA.
  • Passion for palliative care and a holistic, person‑centred approach.
  • Empathy, resilience and outstanding communication skills.
  • Ability to work effectively in a multidisciplinary team.
  • Strong organisational abilities and critical thinking.
  • Post‑graduate studies in palliative care or willingness to pursue them.
