The RN works with the client and their families alongside a multi-disciplinary team to achieve identified goals and outcomes.On any given day, you’ll be conducting specialist symptom management, case co-ordination, prioritising caseloads and will utilise your comprehensive understanding of medications used in the palliative care domain. The service covers a large geographical area that spans both Cessnock and Singleton LGA’s – this means that driving 1-2 hours to a client’s house is part of the RN role.You won’t be working alone. You’ll be supported by the Nurse Unit Manager, CNC and CNS, as well as working collaboratively with the other members of the multidisciplinary team. There will be multiple opportunities for Palliative Care education and professional development on a regular basis. Job-share arrangements may be considered.Where You'll Be WorkingThe Hunter Valley Palliative Care Service is based at Cessnock House, on the grounds of Cessnock Hospital. There are currently eight Nursing staff and one Social Worker working from this office at any given time.The work hours are: 8.30am to 5pm. The service works on a 7-day rotating roster therefore, you will be expected to work on some weekends and be Nurse on Call from 5pm to 8.30am, regularly.As Our Preferred Candidate, You WillDemonstrate your expertise in palliative care within the community setting. Ideal candidates will have completed post-graduate studies in palliative care or possess relevant, hands-on experience. Additionally, the ability to confidently navigate CHIME (Community Health Information Management Exchange) systems is essential for success in this role.
