The Role:

We are looking for compassionate and dedicated professionals to join our team in a blended role as a Care Finder and NDIS Support Coordinator.

This unique dual role offers the opportunity to work across two vital programs:

Care Finder Program: In this role, you will provide specialised assistance to help older adults understand and access aged care supports and connect with relevant community services through the Care Finder program.

NDIS Support Coordination: As a NDIS Support Coordinator, you will deliver individualised support to people living with disabilities, including psychosocial disabilities, helping them maximise the benefits of their NDIS plans.

About you:

We are looking for someone who has:

  • An ability to establish and maintain effective relationships with clients and the community.
  • A passion to build knowledge of the programs and related service providers.
  • Tech savvy with an ability to research and learn about local services.
  • A proactive and empathetic approach with a willingness to learn and upskill.
