    The physician will provide geriatric primary care at the Veteran's home as part of the Home Based Primary Care (HBPC), Patient Aligned Care Team (PACT). The individual will provide clinical care of veterans enrolled in the HBPC program at Michael E. DeBakey VA medical Center and the Community Based Outpatient Clinic (CBOC) satellite sites. The incumbent will serve as a supervisory attending physician for mid-level providers (Nurse Practitioners and Physician Assistants). The incumbent will demonstrate leadership and provide clinical input and oversight for all patient treatment plans in collaboration with an interdisciplinary treatment team. The provider will implement and comply with administrative review of the service's operations to assure compliance with all internal and external survey requirements. Providing clinical management of patients which may include skills necessary for stabilization of acute complications and initiation of basic life support is to be expected. The provider will provide patient education in an interactive setting with good interpersonal skills as well as sign all appropriate documents as administratively required. Items to be signed include, but are not limited to encounters, open consults, referrals for non-VA care, etc. The incumbent will mentor rotating medical students, residents, and other trainees during HBPC rotations and serve as an attending physician for the current Geriatric Fellowship program. The physician will be considered a leader in performance improvement. This position has On-Call responsibilities and provides coverage while others are absent.

    Work Schedule: Monday through Friday, 8:00 am to 4:30 pm; Alternative work schedule may be available.

    Basic Requirements:
    • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with the Department of Veterans Affairs policy.
    • Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
    • Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
    • Must have completed residency training or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
    • Proficiency in spoken abd written English.

    Qualifications
    Preferred Experience: Board Eligible/Board Certified Internal Medicine or Family Practice physician with focus in Geriatric Medicine preferred. Driving required.
