Job Summary

Through direct community outreach, the Community Health Worker (CHW) will be responsible for helping specific target populations navigate and access the healthcare system, community services, other resources, and adopt healthy behaviors. CHWs will work closely with medical providers, faith and community-based organizations, local schools, and other agencies to promote, maintain, and improve health outcomes for the target population. CHWs will provide culturally appropriate health education and information, give social support and informal counseling, advocate for individuals and community health needs, and facilitates evidence-based chronic disease prevention programs. CHWs build trusting relationships with their patients and strive to improve the health of the patients they serve. CHW certification and other specific program trainings will be provided after hire.

Supervisory/Management Responsibilities

This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Education

High School diploma or equivalent 

Minimum Experience

1 year - Experience working in a community-based setting including paid or volunteer activity with a non-profit organization, church, healthcare organization, neighborhood community center or other relevant community-based involvement.

Required Certifications/Registrations/Licenses

SCDL - SC DRIVERS LICENSE - LSCD

Other Required Experience

Experience working in a multi-cultural setting - Preferred

Knowledge of some medical terminology and experience with health information technology - Preferred
