Job Summary

Responsible for the daily operations and supervision of clinical and clinical support staff providing home infusion and ambulatory pharmacy services, Home Health and Home Medical Equipment and Respiratory Services for Home Care. Collaborates with the Home Care Leadership to assure that clinical services and patient care is in accordance with clinical standards of practice and all regulatory and accrediting body agency guidelines. Responsible for ensuring safe and efficacious clinical practice that allows for the ideal patient experience.  

Job Responsibilities

  • Collaborates with the Clinical Managers of home care on the supervision of clinical and clinical support staff providing home infusion and ambulatory pharmacy services, Home Health and Home Medical Equipment and Respiratory Services for Home Care.
  • Responsible for the direct and qualified supervision of clinical and clinical support staff providing pharmacy dispensing and home infusion services for Home Care patients.
  • Responsible for the oversight of pharmacy dispensing services for Ambulatory care patients at the KOP facility.
  • Collaborates with the Clinical Managers of home care on the supervision of the daily operations of the clinical services provided to home care patients allowing for the ideal patient and employee experience. Assures that the coordination of services by the interdisciplinary team promotes and provides safe, efficacious, clinically appropriate, timely and cost effective care and services.
  • Provides supervisory visits of clinical and clinical support staff.
  • Ability to fill the role of direct clinical care provider, pharmacist, as required.
  • Collaborates with Warehouse Ops Supervisor and sub contracted HME.RT vendor to assure  that equipment management services for the home care department  are in accordance with all regulatory and accrediting body guidelines and CHHC/CHOP Policy and Procedures.
  • Assures quality of the home care clinical services provided.
  • Assures clinical practice is in accordance with practice guidelines, policy and regulatory guidelines.
  • Monitors and assures that all clinical documentation is in compliance with Joint Commission, Medicare CMS, CHHC/CHOP policy and procedure and all regulatory and accrediting agency guidelines.
  • Collaborates with Leadership to enhance patient safety and patient care.
  • Collaborates with Leadership to enhance professional development of staff
  • Collaborates with Home Care Leadership on the attainment of the annual operating plan for the Home Care Department.
  • Determines educational opportunities, provides education, oversees implementation of required changes and monitors for compliance.

Required Licenses, Certifications, Registrations

Licensed Pharmacist (PA)

Required Education and Experience

Required Education: Bachelor of Science degree in health care profession.

 

Required Experience: Science degree in health care profession required.

Preferred Education, Experience & Cert/Lic

Preferred Education: Doctorate of Pharmacy

 

Preferred Experience: Minimum of 5 years clinical experience in acute, sub-acute or home care pediatric experience. Minimum of three years’ experience in home care in a managerial position and/or clinical supervisory or lead clinical role.

  • Strong working knowledge of project management methodology.
