*Elara Caring *is seeking a passionate and dedicated *Hospice RN Case Manager* to join our team in the *Battle Creek/Kalamazoo *area*.* As an integral part of our elite team of healthcare professionals, you will have the opportunity to make a meaningful impact by providing exceptional patient care across the continuum of care, ensuring that each patient receives the Right Care, at the Right Time, in the Right Place.

*Responsibilities:*
* Provide high-quality nursing care to hospice patients in their homes, delivering compassionate and patient-centered care.
* Conduct comprehensive patient assessments and develop individualized care plans tailored to meet each patient's unique needs and preferences.
* Administer medications, perform procedures such as catheterization and IV insertion, and provide tube feeding as needed, ensuring the comfort and well-being of patients.
* Utilize electronic health record systems to accurately document patient information and maintain up-to-date records.
* Collaborate with interdisciplinary teams, including physicians, social workers, and therapists, to coordinate and optimize patient care.
* Educate patients and their families on disease management, symptom control, and end-of-life care, empowering them to make informed decisions about their care.
* Coordinate with healthcare providers and community resources to ensure continuity of care and support patient outcomes.
* Ensure compliance with all regulatory requirements and standards of practice in hospice care, upholding the highest standards of professionalism and ethics.
* Participate in ongoing training and professional development activities to enhance nursing skills and knowledge, staying abreast of best practices and evidence-based interventions.



*What is Required?*
* Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
* Current State Licensee as a Registered Nurse RN
* 1 year of experience in a clinical care setting
* Experience in hospice or home health environment is preferred
* Nursing skills as defined by accepted nursing standards
* Ability to handle multiple priorities, documentation requirements, and deadlines
* Reliable transportation to perform job tasks
