Overview:

Admission Responsibilities:
  • For patient admitted directly to inpatient floor, RN collaborates and communicates effectively with primary nurse on patient admission needs
  • Facilitates admission through admission navigator documentation in the EMR
  • Performs first head-to-toe assessment on admission for Licensed Practical Nurses
  • For patients admitted to the ADT suite, RN will complete admission documentation and thorough assessment.
  • RN will provide inpatient care (including medication administration) to patients within the ADT suite until a bed becomes available on an inpatient unit.
  • Completes initial skin assessment
  • Orients patient and family to room, hospital, and nurse call system
  • Provides and documents education to patient and families.


Discharge Responsibilities:
  • Collaborates and communicates effectively with primary nurse on patient discharge needs
  • Provides discharge education to patient and family
  • Ensures that all required admission and discharge tasks are complete prior to patient discharge
  • Removes any applicable devices not indicated for discharge
  • Documents education provided in EMR
  • Coordinates patient transportation for discharge o For patients that have transportation barriers for a timely discharge, RN will move patient to ADT suite, complete discharge education and tasks, and provide care until patient's transportation arrives
  • RN will collaborate and communicate effectively with primary nurse and floor Care Management team to ensure smooth transition to ADT suite
  • Remove bed linens, trash, and equipment from patient room


Transfer/Task Responsibilities:
  • Collaborates and communicates effectively with primary nurse on patient needs
  • Transfer: Coordinates patient transfer to appropriate level of care
  • Tasks (not exhaustive): o Assist with medication administration, including IVP medication for LPNs
  • PIV insertion, including Ultrasound-guided insertion
  • NG placement
  • Urinary catheter insertion
  • Wound dressing changes
  • Patient nutrition and hygiene
  • Answer patient calls and attend to patient needs while rounding


Qualifications:
  • Education: Graduate of an accredited nursing school
  • Licensure: RN licensure in applicable state of practice.
  • Experience: Two or more years peri-operative patient care services or medical/surgical nursing experience in hospital preferred.
