Responsibilities

Position Summary

Case Manager will provide quality substance abuse case management services according to the program's established policies and procedures. Case Manager conducts bio-psycho-social assessments and any required intake/assessment tasks, assist in discharge planning, conducting group sessions and participates in the development of group content.


Qualifications

Essential Duties and Responsibilities

  • May assess perspective patients for treatment, identifying psychosocial, substance use, mental health and medical issues to determine if patient meets admission requirements.
  • May complete utilization review pre-certifications based solely on the thorough assessment completed by qualified intake staff member.
  • Administers breathalyzer and vital signs upon intake and documents accurate results. May be responsible for making the decision to send patients out to the hospital if required.
  • Provides directions or arranges appropriate transportation services as needed to transport patients.
  • Responsible for introducing new patients to therapeutic community.
  • Explains the evaluation and admission process to the potential patient/referral source and informs patient of necessary items to bring for treatment.
  • Documents all intake information into the TIER program including demographics and all appropriate assessments.
  • Maintains daily census and provides assessment for appropriate intakes.
  • Ensures intake paperwork is complete and accurate. Creates intake files. Files intake-related documents.
  • Acts as role model by following program and treatment norms, demonstrating honesty and a willingness to listen to patients' issues and taking responsibility for own actions.
  • Works collaboratively with the Admission Center staff, Utilization Review staff, Nursing Staff and Residential Assistants.
  • Communicates and collaborates with nursing department to facilitate an efficient transition of customer into the treatment milieu.
  • Maintains professional relations and assures information is communicated effectively with staff as well as to patients, families and referral sources.
  • Attends staff meetings as indicated and treatment team meetings when necessary to monitor results.
  • Performs all other duties as required or assigned.

Knowledge, Skills and Abilities

  • Ability to inspire client confidence in the FRN Staff and the FRN Model of Treatment.
  • Ability to consistently demonstrate through conduct and communications the stability required to effectively assist patients and their family members through the FRN Model of Treatment.
  • Excellent verbal and written communication skills required.
  • Extensive and demonstrated knowledge of substance abuse, group process, and discharge planning.
  • Experience in completing assessment and group therapy to adults with addictive and mental health disorders.
  • Experience with facilitating didactic and experiential groups.
  • Knowledge of or willingness to use Motivational Interviewing techniques.
  • Strong verbal and written communication skills and ability to manage high volume of client related paperwork.
  • Must have high level of interpersonal skills. Position continually requires demonstrated poise, professionalism, tact and diplomacy.
  • Exercise good judgment, apply common sense and successfully handle ambiguity.
  • Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization.
  • Excellent organizational skills with the ability to prioritize workload and meet deadlines.
  • Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint). Proficiency in MS Access (or other database program) is preferred.
  • Understands need for and maintains appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources and all other contacts.
  • Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families and referral sources.
  • Knowledge of group process, group facilitation, and conflict resolution.

Education and Experience

  • Undergraduate degree in social work, counseling, psychology or mental health and a minimum of one year of substance abuse experience required.
  • Minimum of one (1) year of supervised substance abuse experience

Work Environment and Physical Demands

  • Sits, stands, lifts and moves intermittently during work hours.
  • Must be able to lift, push, pull and/or move up to 50 pounds.
  • Ability to be physically mobile, climb stairs, traverse the campus on uneven terrain.
  • Position requires substantial work on the computer requiring manual dexterity and visual acuity.
  • Ability to read, write and analyze detailed and complex information.
  • A current, valid driver's license and a clean driving record is required for use of company vehicles.
  • Due to the 24/7 operational demands, must be available after normal working hours and on holidays to manage emergencies that may arise.
