RN Weekend Hospice* Part Time VNA of the Treasure Coast Overview: The Visiting Nurse Association of the Treasure Coast is seeking a RN Weekend to work with Hospice Home Team to provide skilled nursing care in accordance with the VNA Hospice Plan of Care for each patient. As a VNA associate, you will have the opportunity to work with a highly skilled interdisciplinary team providing compassionate, high quality care to patients and their families at end of life in a variety of settings that includes private homes, hospitals and nursing facilities. This dynamic and professionally fulfilling position calls for a high level of autonomy, organization and communication skills. Responsibilities: 1. Identify patient/family needs, including physical, psychological, social and spiritual factors, and provide supportive care in accordance with the attending physician's orders and plan of care and VNA Hospice policies and procedures. 2. Initiate and maintain communication with attending physicians, Patient Care Managers, RN case managers, other Hospice staff members and other agencies, as needed, to coordinate optimal care and use of resources for the patient/family. 3. Documentation in electronic medical record system is completed within agency guidelines and to meet regulatory requirements. 4. Respond to medical or emotional crisis and/or death of patient and make home/facility visit to provide direct care as appropriate. Provide appropriate support at time of death 5. Respond to all referrals as assigned by the PCM; evaluate patient condition and eligibility for Hospice care. Complete admission process per patient/family request if patient is Hospice appropriate. 6. Educate primary caregivers, volunteers and employed caregivers to provide care for patients as indicated. 7. Meet regularly with Hospice nursing staff to review problems or unique issues from weekend hours, share professional support and exchange feedback aimed toward enhancing professional growth. 8. Provide on-call coverage during on one weekend night (Saturday or Sunday) from 4:30 pm - 8:00 am. 9. Participate in VNA Hospice orientation and in-service training programs for professional staff. 10. Position Summary:
The Sonographer I is an entry-level sonographer focused primarily on developing knowledge and skills. Requires consultation with more experienced sonographers and clinicians. The Sonographer I is guided by policy and oriented by task completion when caring for patients and their families. Demonstrates and completes unit-specific Advanced Beginner Clinical Pathway. Must have a minimum of a high school diploma or equivalent. Eligible for specialty registry through ARDMS or CCI. Must have current BLS certification, AHA. The ability to read, write, speak and understand English is required.
The Sonographer II demonstrates an expanding knowledge base in the management of diagnostic testing for specific types of patient populations. The Sonographer II applies more in-depth clinical reasoning and critical thinking skills. Demonstrates an increased capacity to problem-solve through application of testing modalities. Demonstrates and completes unit-specific Competent Clinical Pathway. Must be a graduate of an approved diagnostic medical sonography program with minimum of one-year experience; additional clinical experience or credentials may be considered in lieu of some education. Must have specialty registry through ARDMS or CCI. Current BLS certification, AHA is also required. The ability to read, write, speak and understand English is required.
The Sonographer III demonstrates advanced knowledge and skills in the management of diagnostic testing. Demonstrates focused critical thinking, integrates multi-system implications and provides anticipatory observation and management for predictable events. Demonstrates interdisciplinary collaboration and an outcome-oriented approach to care. Must be a graduate of an approved diagnostic medical sonography program with a minimum of five years’ experience; additional clinical experience or credentials may be considered in lieu of some education. Must have specialty registry through ARDMS or CCI. Current BLS certification, AHA is also required. Dual registries preferred (ex. ARDMS: Pediatric Echo, Fetal Echo, Adult Echo OR CCI: Registered Congenital Cardiac Sonographer (RCCS), Registered Cardiac Sonographer, Advanced Cardiac Sonographer (ACS). The ability to read, write, speak and understand English is required.
Shift: Full-Time, Day Shift, On-Call is required, which can include weeknights & weekends.
***This position is based at the main campus hospital and outpatient care center in Saint Petersburg, FL. There is occasional travel required to additional outpatient care centers in the region. These centers are located approximately 30-45 minutes from the main campus; travel time may vary depending on traffic. If travel is required, time and mileage will be compensated outside of normal time and travel to main campus.
