Responsibilities

Open Sky Community Services, in collaboration with the Department of Corrections (DOC), is excited to announce a first of its kind program in the state of Massachusetts, The After Incarceration Support Program. This program will support the re-entry of returning citizens who have been recently released from the DOC and other individuals who have a history of Justice System Involvement. The Aftercare Incarceration Support Program is community-driven and fosters active community involvement, and engagement. The program offers a safe, judgement free, resource rich environment that breaks down barriers to reintegration while providing access to essential, customized resources, and supports a mission to invest in each individual’s success. The scope of services includes outreach and engagement, food and nutrition assistance, educational and job placement services, housing assistance as well as healthcare and addiction treatment.

The Multi-Services Lead Navigator will provide support, assessment, education, and safety planning for individuals enrolled in Community Services programs. They will also provide operational assistance to programs and services such as carrying a caseload, participating in on call rotation, Electronic Health record entry, facilitating groups, assisting with the operation of programs, and assisting with the day-to-day operations of the program.

Other Key Responsibilities:

  • Engage with individuals served in the program and in the community and support the individuals in gaining access to treatment services and community resources
  • Implement interventions as identified in the Support Plan and document individual progress to maintain a permanent record of activity and services provided.
  • Act as a liaison between individuals served, community agencies and families to coordinate treatment, including employment and education entities, Housing entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others.
  • Provide support to programs within the continuum of care to fill in for vacancies, support program expansions, staff training, enhance quality of care and other support functions.


Qualifications

  • High School Diploma, GED or equivalent, required.
  • Valid drivers license, acceptable driving record and reliable transportation, required.
  • Minimum of 2 years' experience working with homeless adults with serious and persistent mental illness, substance use disorders, and underserved populations.
  • Experience with local resources, navigating health, behavioral health and/or substance abuse treatment systems, strongly preferred.


Compensation details: 22.96 Hourly Wage
