General Statement of Duties:

Supporting Transitions and Reentry (STAR) Program is Essex County Sheriff's Departments new community-based model that relies on the use of validated risk-assessments and enhanced relationships with our community services network to provide participants the support, supervision, and services to thrive in our communities. The STAR team approach allows each participant to feel supported and connected to the Department and respective communities. The STAR Navigator plays a crucial role in the Essex County Sheriff's Departments goals to assist justice-involved individuals in communities and those transitioning back to our communities, along with the ongoing development of community programs that promote public safety initiatives. The STAR Navigator is uniquely positioned to establish a strong and effective working relationship with the individuals they are working with in a manner that is key to the rehabilitative and supportive efforts that are critical to their community success.

Supervision Received:

Work under the supervision of the Assistant Superintendent III - STAR, or other employees of higher grade who provide instruction and guidance, assign work and review performance through oral and written reports for adherence to policy and conformance with rules and regulations of the ECSD.

Essential Functions:

  • Assessment, coordination, support, and shared community supervision of Essex County justice-involved men and women, in conjunction with respective criminal justice partners. The STAR centers provide accountability, support, and intervention to participants that are both referred for services or voluntarily seek support from center from surrounding communities.
  • Responsible for conducting all designated assessments and conjunction with center clinical staff, including but not limited to ORAS-CST (Ohio Risk Assessment System-Community Supervision Tool).
  • STAR Navigator will utilize the results from the ORAS to recommend and coordinate appropriate STAR Tier Matrix', as it relates to engagement and intensity plan, their individual STAR center programming schedule, and applicable community services to target the high-risk domains to reduce risk to reoffend: Criminal History; Education, Employment, & Financial; Family & Social Supports; Substance Abuse & Mental Health; and Criminal Attitudes & Behavioral Patterns.
  • Provide community support, service coordination, and advocacy for all justice-involved STAR participants, as assigned by the STAR supervisor.
  • Actively participate in service planning for justice-involved persons coming out of facilities that will be attending STAR centers.
  • Provide New STAR Orientation for new participants, as part of team approach.
  • Compile assessment data and individual case history information to complete participant biopsychosocial within specific timeframe as indicated by STAR supervisor.
  • Participate in monitoring community accountability via community supervision checks which may include employment, education, and community partner visits.
  • Recommend appropriate interventions for non-compliances in conjunction with clinical staff and STAR supervisor. Respond to need for crisis interventions in conjunction with center staff.
  • Responsible for being primary Justice-partner Liaison (Court, Probation, Parole, Police).
  • Provide thoughtful, detailed participant progress reports at designated periodic reporting periods.
  • Schedule and facilitate transition plan meetings for participants returning to community.
  • Participate in weekly Multi-Disciplinary Team STAR participant reviews.
  • Assist with participant transportation needs or support, as assigned by STAR supervisor.
  • Demonstrate cultural and gender responsiveness while working with diverse populations and communities.
  • Maintain comprehensive progress notes within established timeframes and inputs all documentation into designated information technology systems.
  • Function as a positive role model, while fostering positive relationships with participants, colleagues, and all community partnerships.
  • Develop and maintain professional relationships and open communication with all STAR Stakeholders and partners (Court, Police Departments, HOC's, dually involved State Agencies (DCF, DMH, DDS, DYS), Community Resources, and Participants Attorney.
  • Adhere to established STAR center safety and security standards, protocols, and precautions.

    Position Type/Expected Hours of Work:

    This is a full-time position. Days and hours of work are Monday through Friday (4-day shifts/1 night shift).


Qualifications

Required Education and Experience:

  • Associate's Degree.
  • Minimum of two (2) years experience in social/human services, criminal justice, or mental health/substance abuse with a combination of education, training and experience which provide the knowledge and skills for the assignment.
  • Must be computer literate with a high degree of proficiency and accuracy in typing and data entry.
  • Ability to communicate with both employees and general public, and function calmly under pressure.
  • Demonstrate ability to build strong community relationships and establish trust with our partners and justice-involved participants.
